Re-credentialing is the process we use to periodically review and re-verify your professional credentials in conjunction with First Sun EAP’s credentialing criteria.  In order to be able to continue referring clients to you, we need the following information:

• A copy of your license(s)
• A copy of certification(s), if applicable
• A copy of your malpractice insurance
• Practice Information Form (completed and returned)
• Malpractice Insurance Questionnaire (completed and returned)
• Form W-9 (completed and returned)
• First Sun EAP Provider Payment Checklist (completed and returned)

You may download First Sun EAP forms, complete and return to us either by mail, fax or scan and email to us. Should we not hear from you, we will assume you no longer wish to be a First Sun Network Provider and will discontinue referring clients to you.
